Source: Centers for Medicare & Medicaid Services (CMS), Provider Information,
Ownership and Skilled Nursing Facility Cost Report datasets and the HCRIS
skilled nursing facility cost report files. Each raw manifest records the
exact URLs, edition, retrieval time and SHA-256 hashes. The official source
data dictionaries are retained internally by the publisher.

CMS states that the public reporting data are works of the U.S. government,
in the public domain, and may be reused without permission. Credit CMS.

The data.cms.gov catalog lists U.S. government works as the license for the
Skilled Nursing Facility Cost Report files: https://www.usa.gov/government-works
(currently published at https://www.usa.gov/government-copyright). That page
also notes that government material must not be used to imply endorsement and
that other people may hold publicity or privacy rights.

CMS publishes the Healthcare Cost Report Information System (HCRIS) files on
its cost reports page without a separate license; as federal agency
publications they follow the U.S. government works rules above. The page
carries this disclaimer: "Derived conclusions and analysis generated from this
data are not to be considered attributable to CMS or HCRIS." It also states
that CMS is not responsible for data that is misrepresented, misinterpreted or
altered. This raw snapshot contains independently copied, manifest-verified facility,
ownership, chain and public-use cost-report tables, their upstream manifest,
the HCRIS source page, original selected HCRIS ZIPs and Form 2540-24 form and
worksheet instructions. Original PUF source files, methodology, dictionary
and HCRIS documentation archive remain in the upstream nursing-home capture;
they are not duplicated here.

This collection contains published facility, ownership and cost report
records. It does not contain patient records, government logos or third-party
imagery. Public availability of a contact does not establish consent to
marketing. No exclusive rights in the underlying government facts are asserted.

Modifications: normalized column names, whitespace, typed dates, numeric and
boolean representations, telephone formatting, exact-CCN joins and chain
aggregates. Cost report values keep their published amounts; we added ISO
dates, HCRIS report status and dates linked by report record number or exact
CCN and period, form-specific Medicare and Medicaid days and a Medicaid day share,
exact-CCN facility links and a most recent report per facility. These derived
fields are our analysis, not statements by CMS. Original HCRIS bytes and imported normalized facts remain here; original
PUF bytes are held in the upstream capture. Both retain source-row provenance.


PRODUCT SCOPE

No Overture enrichment is delivered. Individual owner names are withheld.

32 for net income; CMS-2540-10 uses line 31. Form identifiers, source namespaces,
periods and balance-sheet bases are preserved. Ratios and cohort medians are our
analysis, not CMS conclusions or assessments of financial distress.

Form 2540-24 SNF FFS days are on S-3 Part I line 1 and HMO days on line 2.
Combined payer totals require both reported components; missing cells stay
unknown. Form 2540-10 retains its separately defined program-day basis.
