SOURCE RIGHTS AND ATTRIBUTION

Source: U.S. Department of Labor (DOL), Employee Benefits Security
Administration (EBSA), Form 5500 Series public datasets: the "Latest" Form
5500 and Form 5500-SF main tables and the Latest Schedule A (insurance
information, including Part 1 agent and broker lines), Schedule C (Part 1
line 2 service providers and their service codes), Schedule H and Schedule I
tables for one DOL form year, with their published layouts, data dictionary,
dataset guide and form instructions. The raw manifest records the form year,
URLs, retrieval times, DOL extract date and SHA-256 hashes. The form year and
extract date appear in README.txt.

DOL states that information it creates is generally in the public domain and
may be reproduced without permission, encourages attribution to the U.S.
Department of Labor, and does not permit its seals or logos to suggest
endorsement. Form 5500 returns and their schedules are filed by plan sponsors
and administrators and are open to public inspection; the instructions state
that schedule contents are public information subject to publication on the
Internet, and DOL publishes these tables for public disclosure. This dataset
keeps factual plan, sponsor, insurance and service provider fields only. It
does not include other schedules, attachments, filing images or other
submitted documents.

PERSONAL DATA

Fields that name natural persons are withheld from the delivered tables and
remain only in the captured source evidence: signer names for the plan
administrator, sponsor and DFE (electronic and manual signatures), in-care-of
names, plan administrator names, paid-preparer contact blocks and trustee or
custodian names and telephone numbers. Administrator names are withheld because
they mix individuals and organizations and cannot be separated reliably; the
administrator EIN, address and telephone number are retained as published.

Insurance carrier, agent and broker, and service provider names are delivered
only when the name itself shows an organization: a legal-form word (such as
Inc, LLC or Company), an organization word (such as Insurance, Benefits or
Bank), or, for a carrier, an assigned NAIC company code. Names with contact or
proxy wording, broker and provider names that set off a separate personal-name
segment, Schedule C entries coded as an individual trustee or employee, and
all other names are withheld with their street lines, fee purposes and
relationships; their amounts, codes, EINs, city, state and ZIP remain.
Schedule A narrative explanations are not delivered. Delivered business names
can include an owner's name, as an agency named after its founder does.

Sponsor names are business names reported on the return. A sole proprietor's
business name can include a person's name. Published addresses and telephone
numbers are filer-provided plan contacts; their publication does not establish
consent to marketing, reachability or current accuracy.

MODIFICATIONS

Normalized column names, whitespace, ISO dates, ZIP and telephone formats,
decoded plan characteristics and DFE codes, feature flags, latest-filing flags,
sponsor aggregates, and NAICS titles decoded from the business code. Harmonized
columns combine the two forms and the equivalent Schedule H or I lines; a line
absent from a form or schedule stays blank. Schedule A benefit types,
organization codes and Schedule C service codes are decoded with the form
instructions' labels, service roles are grouped from service codes, premium
lines and compensation amounts are added where stated, and carrier, broker and
provider names follow the organization-name rule above. Original source bytes
and source-row ordinals remain available for audit.


PRODUCT SCOPE

Reviewed 2026-10-03. Inputs are independently copied manifest-verified normalized DOL tables. This product excludes place contacts and sibling enrichment. It selects latest plan-year contracts with reported health, dental or vision benefits and their broker lines, preserving original source lineage and all broker withholding statuses. Contract premiums remain bundled; filing context is joined only by exact ACK_ID. It contains no submission attachments, government logos or proprietary contact feeds.
